How to set up the User Profile like PokerDIY (Australia)

This is the template for user profiles on PokerDIY (this is my user). You can see the end result on my player profile. My basic requirements were not to expose the email address, and to integrate with the rest of the Smart-Thinker product suite as well as Ventrians Private Messages. I also added some additional user information and fields to the core profile:
